Three trends shaping Canadian physiotherapy in 2026
Pelvic health is no longer niche. Cash-pay is growing. And the demand for evening hours is reshaping clinic schedules across the country.

A few patterns are visible in the data this year. Some are obvious to anyone running a clinic; others are less so.
1. Pelvic health has gone mainstream
Five years ago, pelvic health was offered by a handful of specialised clinics. Today it's table stakes. Patients now expect their general clinic to either offer pelvic health or refer to one that does — and they expect that referral to be specific.
2. Cash-pay is quietly growing
More patients are paying out-of-pocket and expensing through health spending accounts than billing insurance directly. This is changing how clinics design intake — and how they price.
3. Evening hours are no longer optional
Clinics with consistent evening availability fill those slots first. The 9-to-5 clinic is increasingly the exception, particularly in dense urban neighbourhoods.
